Every home visit depends on work that begins long before a care professional reaches the door. Patients need to be onboarded, treatment plans coordinated, teams assigned and information made available to the people involved.

For a healthcare provider managing this activity across multiple systems, NineDots defined a product concept to bring the care journey into one connected framework.

We mapped the workflow from patient intake and treatment planning through home visits, clinical documentation and treatment summaries. The specification connected these steps with the operational processes behind them, including treatment packages, service providers and billing requirements.

The concept paired a web workspace for coordination and management with a mobile experience for professionals in the field. Each addressed the needs of its users while contributing to the same patient journey.

Our work translated a complex service into a clear product definition: how information should move, what each team needs to see and how the different stages of care should fit together.

The vision was connected care, supported by a product shaped around the people who organize it and the people who deliver it.
